Terms and Conditions

Welcome to Adaptive Physiotherapy & Massage. By booking an appointment, enrolling in a membership, or using our services, you agree to the following terms and conditions. These are designed to ensure clarity, fairness, and the highest standard of service.

1. General Information

2. Bookings and Appointments

Booking Requirements

Appointments can be booked online or by phone. All new clients must complete an intake form before treatment.

Arrival Time

Please arrive at least 5 minutes prior to your scheduled appointment. Late arrivals may result in a shortened session to avoid impacting the next client’s appointment.

3. Cancellation & Missed Appointments Policy

We understand that life happens. However, late cancellations and missed appointments affect our ability to help others.

  • Less than 4 hours’ notice or missed appointment:
    A $50 cancellation fee will apply.
    This is payable within 7 days, either at your next visit, over the phone, or online.

  • Exceptional circumstances:
    We understand emergencies occur. These situations will be reviewed on a case-by-case basis.

By attending an appointment, you acknowledge and agree to abide by this cancellation policy.

4. Massage Membership Terms

Our Massage Membership provides regular treatments at a reduced rate. As a member, you agree to the following:

  • Ongoing billing: Membership fees are charged weekly or monthly, depending on your selected plan.

  • Cancellation of membership:

    You may cancel your membership at any time by submitting the Membership Cancellation Form (available online).
    Please note that a minimum of 2 months’ notice is required.

    During this 2-month notice period:

     

    • Your membership remains active and you may continue to use your included services.

    • Any unused massage credits will expire at the end of the notice period and cannot be carried over or refunded.

  • Pausing membership:
    Memberships can be paused for a minimum of 2 weeks and up to 8 weeks per calendar year. Notice must be provided at least 5 business days in advance.

  • Missed appointments:
    If you miss a scheduled session without appropriate notice, it may count as a used session unless otherwise agreed.

5. Health Information & Treatment Consent

All clients must provide accurate and complete health information. This ensures your treatment is safe and appropriate. You agree to update us with any changes in your condition.

By attending a session, you consent to the treatment provided by our physiotherapists, exercise physiologists, and/or massage therapists.

6. Payment Terms

Payment is due at the time of service unless prior arrangements have been made. We accept EFTPOS, credit card, and approved third-party funding (e.g. Medicare, DVA, NDIS, WorkCover).

  • Unpaid Fees
    If payment is not received, services may be paused until the outstanding balance is cleared.

  • Late Payment Fee
    A $15 Late Payment / Declined Credit Card Fee will be charged on any overdue payment or failed transaction.

 

Please ensure your payment details are kept up to date to avoid interruptions to your care.

7. Privacy Policy

We are committed to protecting your privacy. All client information is kept confidential in accordance with the Privacy Act 1988 (Cth) and relevant health privacy laws.

Your data will never be shared without your consent, unless required by law.

8. Liability Waiver

While our team takes every precaution to provide safe and effective treatment, clients participate in services at their own risk. Adaptive Physiotherapy & Massage and its practitioners are not liable for any injury, loss, or damage sustained during or after a session, except as required by law.

9. NDIS, DVA, and Other Third-Party Funders

Clients receiving services under NDIS, DVA, or WorkCover must ensure a current referral or service agreement is in place. Missed appointments may not be claimable and may result in out-of-pocket fees.

11. Medical Treatment Consent

I consent and authorise representatives of Adaptive Physiotherapy & Massage to administer or seek medical assistance on my behalf in the event of an accident, illness, or medical condition occurring while I am attending an appointment or participating in any service or program. I agree to cover any associated costs incurred in obtaining such medical assistance.

12. Photography and Use of Images

Photographs and videos may occasionally be taken on site for use in Adaptive Physiotherapy & Massage promotional materials, including social media, website content, and print marketing.

By agreeing to these Terms & Conditions, I grant Adaptive Physiotherapy & Massage the right to use images or recordings that include me, without compensation. All media remains the property of Adaptive Physiotherapy & Massage and may be used for lawful promotional purposes.

If you prefer not to be included in any media, please notify our team in writing.

13. Changes to Terms

We may update these Terms & Conditions from time to time. Clients will be notified of any significant changes via email or through our website.
